Social Services

Social workers can play a large role in a patient’s treatment and recovery. As members of our home care team, Fidelity Health Care social workers offer education and training for patients and families, patient consultation, and staff support. 

Our Social Worker Services:

  • Assessment of social and emotional factors related to the patient’s illness, need for care, response to treatment, and adjustment to care
  • Assessment of the relationship of the patient’s medical and nursing requirements to his or her home situation, financial resources, and availability of community resources
  • Appropriate action to obtain community resources to assist in resolving the patient’s problem
  • Counseling services required by the patient. Counseling of patient’s family is covered only when such services are incidental to other covered medical social services being provided to the patient and when they are reasonable and necessary to treat the patient’s injury or illness
  • Crisis services

Obtaining a Referral

Typically, to receive social services, a patient must have a referral. For home health care services, this requires demonstration of a clear connection between the social and emotional needs of the patient and the patient’s medical condition or rate of recovery. A referral to social work may be requested by the patient’s physician, discharge planner, physical therapist, occupational therapist, speech therapist, family members and/or caregiver, community organizations, or the patient. Commonly, social work referrals are identified by the primary nurse or the intake department. A physician’s order is always required for social work referrals.
