Our team of social workers helps patients and families solve life issues that may arise as a result of illness, injury, or advancing age. Each of Fidelity Health Care’s social workers holds a master’s degree and is licensed in Ohio. Our experience makes us uniquely qualified to help patients and their families.

How Can a Social Worker Help You?

Here are some of the most common ways that social workers provide help. During visits in your home we can help with:

Long-term care planning:

  • What will happen to dad if mom has a health crisis?
  • Mom can no longer care for herself. What options are available?
  • Reviewing care options when disabled patient’s health deteriorates

Daily living:

  • How to obtain meals when patients can no longer cook for themselves.
  • What transportation is available within the community when patient can’t drive?
  • What other resources are available?

Finances and insurance:

  • Identifying financial services that are available to patient
  • Help with navigating insurance and/or Medicare

Palliative care:

  • Helping patients with a chronic illness learn how to manage their symptoms

    End-of-life care planning:

  • Counseling patients, family, and caregivers to make end-of-life not so scary
  • Assistance in getting a patient enrolled in hospice care
  • Support if a patient does not want hospice care

Making connections:

  • Understanding what resources are available and how to tap into them. Examples include Meals on Wheels, home health aides, life line alert, medication dispensers, medical equipment, support groups

How Can You Request a Social Worker?

If a Fidelity Health Care nurse is providing care within your home, that nurse can arrange for visits from a social worker. Ask the nurse to make those arrangements for you. If you do not have in-home nursing care, your doctor can arrange for a social worker to visit your home to provide the help you need.
