Nursing care may be provided by either registered or licensed practical nurses. Care in your home includes service that requires the skills of a nurse. Service, ordered by your physician, may include, but is not limited to, assessment, treatment and education for any medical or surgical condition. The nurses work closely with the patient’s physician to determine the best care plan and with the family to ensure that the patient receives care after the nurse leaves. Educating the family is an integral part of home care and must be maintained to enable patients to gain maximum effectiveness from their home care treatment.

  • Visit care for assessment, education, disease and medicine management

    • Psychiatric: home visits for psychiatric evaluations, counseling, and referrals, medication management
    • Diabetes Care
    • Cardiac
    • Respiratory
  • Post operative care
  • Wound/Ostomy care specialists on staff: the evaluation and treatment of patients with ostomies, draining wounds, fistulas, and other major skin problems
  • Telehealth—Monitor vital signs at home with the use of modern technology web camera.
