Social services are provided on a visit basis by medical social workers to help you network within the community and meet your individual medical and personal needs.

  • Do you or a family member need assistance with obtaining needed medication, food or shelter?

  • Does the person's care create a strain on family members?

  • Are you a person in need of community resources or information to manage short and long term care at home (e.g. financial aid, transportation, counseling, and/or entitlement programs – social security, disability)?

  • Do you require an advocate to secure your rights to benefits and services?

  • Are you a person who is having difficulty managing your care at home?

Description:

The Medical Social Worker assists in understanding the significant social and emotional factors related to the patient’s health problems and ability to follow treatment plan.  Assess the social and emotional factors in order to estimate the capacity of the patient and his family to cope with problems of daily living.  Helping the patient and his family to understand, accept and follow the recommendations and services that are planned to restore the patient to optimum social and physical adjustment within his capacity.  Mobilizing and helping the patient and his family to utilize resources such as family and community to resume life or learn to live within his disability.

Social workers help people function the best way they can in their environment, deal with their relationships, and solve personal and family problems.  They often see clients who face a life-threatening disease or a social problem.  These problems may include inadequate housing, unemployment, lack of job skills, financial distress, serious illness or disability, substance abuse or antisocial behavior.

Through direct counseling, social workers help clients identify their concerns, consider effective solutions, and find reliable resources.  Often they refer clients to specialists in services such as debt counseling, childcare or eldercare or public assistance.  Social workers then follow through with the client to assure that services are helpful and that clients make proper use of the services offered.  Social workers may also review eligibility requirements, help fill out forms and applications and provide support during crises.
