A New Concept...

Fidelity Health Care has combined traditional home care services with the latest in video technology to create a powerful tool in cardiopulmonary disease management.  Every year 65 billion dollars are spent managing the care of patients suffering from these diseases, the majority of the cost coming from multiple ER visits and acute care readmissions.  We believe the combination of home care services and telehealth will provide these patients with the necessary skilled teaching and monitoring to give them the best opportunity for success.

What is Telehealth?

Telehealth is the latest in interactive home telemonitoring.  Telehealth offers a system that features live video/audio and medical peripherals such as:

·        Stethoscope

·        Blood pressure meter

·        Scale

·        Pulse oximeter

·        Digital imaging

All telehealth visits are performed by a registered nurse.

 How does it work?

A patient station is placed in the patient’s home while the central station is located in our office.  The nurse uses the central station to collect information from the patient station via a video visit by calling the patient.  They can see and hear each other while the clinician obtains visual and verbal information, gathers patient vital signs and takes snapshots of areas of interest.  This system functions over ordinary phone lines.

 What is included in the telemonitoring visit?

The telehealth nurse will make a weekly video visit or more frequently if needed.  The patient will be able to call the telehealth nurse M-F during regular business hours.  The nurse will ensure compliance with medications and physician’s orders.  The nurse will perform a head to toe assessment and monitor for any symptoms.   Each visit will include monitoring of vital signs, weight, heart sounds, lung sounds, mental status, emotional status, diet recall, edema, pulse oximetry and any other objective findings as desired by the physician.  The nurse will report any changes or deterioration to the physician immediately and provide a status update as directed by the physician.

·        Identify and implement early intervention to decrease ER visits and hospital readmissions

 The Cardiopulmonary Program…

Our Cardiopulmonary Program consists of traditional home care visits by a registered nurse combined with a period of home telephonic or televideo monitoring.  These monitoring services are called telehealthcare.

We have designed five and ten visit protocols for the home care visits.  These visits provide the intensity of service needed immediately following an acute exacerbation, and as the patient becomes more stable, our home visits become less frequent.  The telehealth visits continue when the traditional on site visits are completed.  This program can be modified to meet the needs of the patient or physician. 

How is telehealth implemented?

A telehealth nurse will visit the patient and family to discuss installation procedures and ongoing monitoring details.  The nurse will obtain written informed consent.  The patient and caregiver will be trained on the equipment. 

What patients should I consider for your cardiopulmonary program?

We are focusing our efforts on the CHF/COPD population, if your patient has experienced the following: 

·        Two hospitalizations in the last six months

·        Two ER visits in the last two months

·        Frequent office/clinic visits

·        Patient with an able and willing caregiver

·        Grounded electrical

·        Standard telephone service

 What could exclude my patient from the Telehealth program?

  • Unwilling
  • Unstable condition needing inpatient management
  • Unsafe
  • Unable/No caregiver
  • No standard telephone service
  • Electrical in home is not grounded
