Disease Management
The Fidelity Health Care disease management teams provide in-home care and education focused on your specific disease or condition. Our board-certified nurses work as part of a collaborative team that also includes therapists and social workers — all working closely with your doctor to help you return to independent living, when possible.
Our in-home disease management programs:
- Educate you, along with your family members or caretakers, about your medical condition, the medicines you’re taking, and how to manage your symptoms
- Help you toward independence through health coaching, including help with diet, nutrition, and exercise
- Focus on providing high-quality care to help you manage your condition, control your symptoms, and return to functional independence as soon as possible.
