Cardiopulmonary disease refers to problems that affect both your heart and your lungs, two body systems that work closely together. Some patients, for instance, may have congestive heart failure in addition to a lung problem, such as emphysema or chronic obstructive pulmonary disease (COPD).

To help you manage your cardiopulmonary disease and its symptoms, the Fidelity Health Care team delivers expert, compassionate care in the privacy and comfort of your own home, working to help you return to functional independence and self-care. Our skilled staff will help you understand your condition and the treatment we’re providing. We’ll also teach you — and any family members or caregivers — about the medicines you’re taking.

Is In-Home Cardiopulmonary Care Right for You?

If you’ve been diagnosed with both heart and lung problems, you could benefit from in-home care if:

  • You have had a recent hospital visit related to your heart and lung conditions and need help transitioning back to your home
  • You are home-bound by your health problems and have difficulty getting out to see a doctor
  • You are on oxygen at home

Who Will Come to Your Home?

Your in-home cardiopulmonary treatment will be managed by a nurse working closely with your doctor. Your treatment team could also include one or more of the following professionals:

During the visits, the nurse will assess your physical condition and discuss any symptoms that might be bothering you. The nurse will also talk with you and your family members or other caregivers about your medications, possible side effects to watch for, and any other concerns or questions you might have.

A respiratory therapist will complete a clinical respiratory assessment and discuss triggers, as well as review home oxygen and other respiratory equipment in your home.

How Long Will You Need to Have In-Home Care?

Every patient is different, and the length of time you’ll need care depends on several things, including how severe your symptoms are, how well you’re responding to treatment, and your doctor’s orders.
