Wound and Ostomy Care

Our certified and highly skilled wound care nurses specialize in assessing and treating wounds and providing wound care recommendations. Working closely with your doctor(s), the team will deliver compassionate care in the comfort of your own home. If needed, the team will also train your family member or regular caregiver so that they can help you, too.

Who Can Benefit From Wound Or Ostomy Home Care?

There are many reasons why people might require wound or ostomy home care. You might have:

  • Acute/chronic wounds (challenging wounds)
  • Skin tears and other skin issues
  • Post-operative/surgical wounds and incisions
  • Traumatic wounds and burns
  • Ulcers (such as arterial, diabetic, pressure and venous stasis)
  • An ostomy or fistula

What Services Do We Provide?

Our wound team can help with:

  • Wound and ostomy care, including advanced care treatments
  • Treatment recommendations
  • Assessing the need for equipment such as a hospital bed, mattress, and other pressure-reducing items
  • Nutritional assessment
  • Obtaining supplies for wound care
  • Coordination and communication with physician(s)
  • Improved monitoring and outcomes

How Often Will the Nurse Come To Your Home?

Our wound team’s goal is to help you and/or your family members or other caregiver learn to care for your wound or ostomy on your own. The nurse will come out more frequently at the beginning of your home care, and the visits will become less frequent as you and your family or caregiver become more comfortable with handling your care.
